The Integrated HIV Prevention and Care Guidance, including the Statewide Coordinated Statement of Need, CY 2027-2031 requires an HIV Prevention, Care, and Treatment Resource Inventory. The purpose of this tool is to support the collection and analysis of all HIV resources in the jurisdiction. The table can be incorporated into the Integrated Plan submission.

A checklist is required in the submission of the Integrated Plans for 2027-2031. Use this simplified, fillable checklist to indicate which elements required new content and which elements were developed as part of another jurisdictional plan.

This template provides jurisdictions with a dynamic tracking tool to centralize and operationalize their Integrated Plan and is intended to be used as a "living document" to reflect the most up-to-date progress.
